- For routine exams, order may be faxed to (325) 232-7814 and our staff will schedule appointment with patient, or you may call (325) 692-4410.
- For STAT or same day exams, please call (325) 692-4410.
- All exams require a signed order. If a S4L order is not available, any order form or prescription will be accepted.
- All
results will be faxed to number provided by ordering entity.
